Few topics in the AAS world carry as much inherited bad advice as aromatase inhibitors. One camp says every cycle needs an AI from day one. Another says AIs are poison and nobody should touch them. Both camps are defending dosing habits, not reading labs. The correct answer is boringly empirical: an AI is a tool to manage a measured, symptomatic problem, nothing more.

The forum protocol era left a trail of crashed E2 and ruined cycles. The old rule of thumb, 1 mg anastrozole per 250 mg testosterone per week, was built on a single case report from the 1990s, not on population data. That dose nukes E2 in most men within 48 hours. The result: a week of joint pain, zero libido, and a lipid panel that looks like a heart attack waiting to happen. The guys who defend that protocol never pulled labs two weeks in. E2 above 60 pg/mL on a sensitive assay, drawn at the same time of day, in two draws at least a week apart, plus symptoms. One draw is not enough: assay variability plus draw timing can swing E2 by 20%.

Two high E2 draws are the standard, but the details matter. Draw at the same time of day, ideally fasted, and use the same lab. The sensitive assay (LC-MS/MS) is the only one worth trusting; the older immunoassay can read 20-30% high, especially when total T is supraphysiological. If you are on a long ester like enanthate or cypionate, draw at trough, right before your next injection. If you are on a short ester like propionate, draw 24 hours after your last pin. And do not draw the day after a heavy leg day or a night of drinking; both can transiently spike E2 by 15-20%. One high draw plus symptoms is a reason to recheck, not to dose. Two high draws plus symptoms is the green light.

Symptoms That Match

Physical signs are the signal: puffy/tender nipples, an unexplained BP rise, or Water-weight gain with tight skin. "I feel emotional" alone is not, because on-cycle moods have a dozen drivers. The classic yes is the combination of tender nipples plus rising E2.

Tender nipples are the most reliable symptom, but they are not always E2. Prolactin spikes from nandrolone or deca can cause the same sensation, and so can a sudden rise in total T without a corresponding E2 increase. If your nipples are sore but your E2 is 40 pg/mL, check prolactin and free T before touching an AI. Also, water retention and BP rise can come from high aldosterone or cortisol, not just E2. The classic mistake is treating every side effect with an AI when the real culprit is dose, sleep, or stress. Keep a symptom log for a week before you draw. If the pattern is consistent, then the lab will confirm it. If the pattern is random, the AI will not fix it, and you will add a new set of problems.

Getting the Dose Right Is a Licensed Medical Professional's Call

AI dosing is not a self-directed titration exercise. The difference between a dose that controls symptoms and one that crashes E2 is a fraction of a milligram, and getting it wrong in either direction has real consequences. This is a decision for a licensed medical professional working from your actual labs, not a forum protocol.

Whatever the starting point, the 10-14 day recheck window matters and is worth understanding regardless of who's managing the dose: anastrozole reaches steady state in about a week, and its effect on E2 lags another few days behind that. Rechecking at day 5 catches only a partial drop, which is exactly when panic and premature dose increases happen — and that is how E2 gets crashed by day 10. If E2 comes back below 15, the right move is to stop the AI and let it clear rather than trying to "rescue" with a different dose; the half-life of anastrozole is about 46 hours, so most of its effect resolves within a week, and joint pain and low libido fade on their own during that window.

Non-aromatizing compounds like Primo, tren, or var need no AI by definition, so adding one there is a category error. Prophylactic AI to "protect against gyno" fails too: gyno risk tracks estrogen receptor sensitivity plus E2 levels, and the intervention is blood-work-guided, not fear-guided.

Prophylactic AI use is not just unnecessary; it is actively harmful to your cycle's anabolic environment. E2 is a potent anabolic hormone in its own right, upregulating IGF-1 receptors in muscle tissue and supporting nitrogen retention. Crashing it with an unnecessary AI trades muscle-building signal for nothing. On a blast, you generally want E2 in the 30-50 range for growth, not the 10-20 range that bodybuilders chase for a 'dry' look. That dry look comes at the cost of joint health, mood, and HDL. If you are running a compound that aromatizes heavily, like testosterone or dianabol, monitor E2 and intervene only when symptoms and labs align. Your muscles will thank you.

When E2 falls below 15 pg/mL with total T at 2,000+ ng/dL, you get a predictable cluster of problems: joint pain, especially in the knees and elbows, depression and anhedonia, zero libido despite high androgens, terrible lipids, and erectile dysfunction. These symptoms get blamed on "the cycle" or "low quality gear" while the AI keeps being taken. The fix is stopping the AI, not changing compounds.

The switch between AIs is not a free choice either. Aromasin's irreversible binding means it does not cause the E2 rebound that anastrozole can when a dose is missed — that rebound is a real phenomenon: anastrozole's reversible binding means E2 can spike 24-48 hours after a missed dose, which is why some guys feel hot flashes on their off days. Aromasin avoids that, but it is also harder on lipids. Letrozole is the last resort for severely elevated E2 with symptoms like gynecomastia or uncontrolled BP, and even then only for a short, closely monitored course before stepping back down — exactly the kind of judgment call that belongs with a licensed medical professional, not a self-managed swap.

The TRT Version Is Different

On TRT (100-200 mg/week), the AI conversation is stricter: E2 rarely needs pharmacologic intervention, and if it does, the dose of testosterone, not the addition of an AI, is usually the thing to adjust. AI-on-TRT is a specialist conversation, and "my TRT clinic put everyone on 1 mg adex EOD" is a red flag about the clinic, not the patient.

On TRT, the AI conversation is almost always a dose conversation in disguise. If your E2 is 70 pg/mL on 150 mg/week, the first move is to drop to 120 mg/week and recheck in six weeks. That single change often brings E2 down to 40 without any AI. The problem is that many clinics use a one-size-fits-all protocol: 200 mg/week plus 0.5 mg anastrozole twice a week. That protocol keeps E2 in range on paper, but it also keeps total T at 1,200 ng/dL and free T at the top of the range, which is not TRT, it is a mild cycle. If you are on TRT and your clinic has you on an AI, ask for a trough total T and E2. If total T is above 1,000 ng/dL, the AI is treating the clinic's overprescription, not your physiology. Lower the T dose, drop the AI, and you will likely feel better with fewer side effects.
