In 4-6 weeks it can: double liver enzymes, crush HDL by 40-60%, raise blood pressure 10-15 points, and shift hematocrit 3-4 points. Nothing on a Dbol cycle is subtle: which makes it the most monitorable compound in the oral family. Weekly draws are not paranoid; they are the price of admission.

Methandienone (Dianabol, Dbol) built the golden era and still tops the "first oral" list: which is exactly why it deserves a blood-work-first article. Dbol's effects are dramatic by design: 17-alpha-alkylated for oral survival, strongly aromatizing, and fast-acting within days. Dramatic in, dramatic on your panel. Before you even touch the blister pack, you need a full lipid panel, CBC with differential, liver panel including GGT, and a week of morning BP readings. If your HDL is already under 35 or your ALT is above 50, Dbol is the wrong first move. You might get away with it, but you will not know if the damage came from the compound or from a poor baseline. The first week on Dbol does not show much on paper. You will feel the androgen buzz and the pumps, but ALT and HDL are still moving. That is why the week-2 draw is the first real signal. The critical interpretation: on a training population, some of that rise is muscle leak (lifting damages muscle, muscle releases the same enzymes), not liver. GGT is the discriminator: GGT elevation is hepatic-specific; isolated ALT/AST with normal GGT is often training artifact.

GGT is the liver-specific marker that separates muscle leak from true hepatocyte stress. ALT and AST rise from training alone, especially with heavy leg days or high-volume work. GGT does not. If your GGT climbs above 40 U/L while ALT is 100, that is a liver signal, not a gym artifact. Bilirubin also matters: if total bilirubin goes above 1.5 mg/dL and your eyes start to yellow, stop the compound immediately. That is cholestasis, not just enzyme elevation. TUDCA can help but it cannot reverse a bilirubin spike. The practical rule: if ALT is 3x baseline at week 2, cut the dose in half or end at week 4. If GGT is elevated at all, end now.

HDL: The Dbol Massacre

Dbol is among the worst HDL-suppressing compounds in common use: the C17-AA structure activates hepatic lipase while aromatization strips estrogen's protective effect. HDL drops of 40-60% within 3-4 weeks at 30 mg/day are typical: a 55 mg/dL baseline sitting at 20-25 by week 4. ApoB and LDL-particle count rise in parallel. Six weeks of Dbol produces a cardiovascular risk profile that takes 8-12 weeks post-cycle to normalize.

The HDL crash is the most predictable number on Dbol, but the LDL side deserves equal attention. Expect LDL to climb 20-40 mg/dL from baseline, and more importantly, the particle pattern shifts toward small dense LDL. That is the atherogenic kind. ApoB tracks this better than LDL-C alone. If your ApoB was under 80 and it jumps to 110 by week 4, that is a meaningful change. You cannot out-supplement this with fish oil or citrus bergamot. Those help, but they will not hold back a 40% HDL drop. The only lever is dose and duration. If HDL is under 25 at week 4, the last two weeks are not worth the cardiovascular debt.

BP and the Estrogen Cascade

Dbol aromatizes strongly: 30 mg/day can raise estradiol into the 60-100 pg/mL range. High E2 + high androgen drives water retention, plasma volume expansion, and a 10-15 point systolic rise in most users. The result is the classic Dbol look (full, puffy, strong pumps) and the classic Dbol risk (hypertensive strain on the same kidneys processing everything else).

Blood pressure on Dbol is not just about water. The combination of high E2 and high androgen increases renin-angiotensin activity and vascular tone. A 10-15 point systolic rise is typical, but some guys see 20+ if they are already on the edge. You should be taking BP daily at the same time, after sitting for five minutes, arm at heart level. If systolic hits 140 at rest, that is the moment to consider an aromatase inhibitor, but only if E2 is actually high. If E2 is under 40 and BP is still high, the problem is volume and vascular stiffness, not estrogen. In that case, reduce sodium and add 2-3 liters of water as your own first moves; if that is not enough, a blood pressure medication like telmisartan is a licensed medical professional's call, not something to source and self-administer. Do not just accept the number.

Weekly Draws for a 6-Week Run

Baseline (Before First Tablet)

Lipids + ApoB, ALT/AST/GGT, CBC, BP log started. Dbol moves fast: without a baseline, week-2 numbers are uninterpretable.

Baseline numbers are your only reference. If you do not have them, you are flying blind. For example, a normal ALT of 40 is fine, but if your baseline is 25, a rise to 60 is a 2.4x increase that might get missed if you only look at the absolute value. Same with HDL: a baseline of 60 dropping to 30 is a 50% cut, while a baseline of 45 dropping to 30 is a 33% cut. The interpretation changes. Also, if your baseline BP is already 135/85, starting Dbol is a bad idea. You will not have room to tolerate the expected rise. Get everything in order first. That means lipids, liver, BP, and hematocrit under 48. If any of those are off, fix them before you start.

Week 2

ALT/AST/GGT + BP check. Liver enzymes are the early tripwire: if ALT is already 3x ULN at week 2, the dose is too high or the run should end at 4 weeks instead of 6.

At the week-2 draw, you are looking for trends, not absolutes. A 1.5x ALT rise from baseline is expected and manageable. If ALT is already 3x the upper limit of normal, that is a red flag. The dose is too high, or your liver is sensitive to the C17-AA group. You have two options: drop to 20 mg/day and recheck in two weeks, or end the run at week 4 instead of 6. Blood pressure at week 2 matters just as much. If systolic is up 15 points and you are already at 135, you are heading toward 145 by week 4. That is the point where most coaches pull the plug. The good news is that Dbol's short half-life means you can stop and recover quickly, but the lipid and liver damage takes weeks to reverse.

Week 4

Lipids + CBC + E2. HDL trajectory decides the rest of the run: below 25 with two weeks left on the plan means the plan shortens. Hematocrit has typically risen 2-3 points by now.

By week 4, hematocrit is typically 2-3 points above baseline, but that number is not the whole story. You also need to look at hemoglobin and red blood cell count. If hematocrit is 51 or higher, you are in the danger zone for viscosity and clotting. You can donate blood, but make sure you are not anemic and that your ferritin is not already low. Dbol also increases E2, and by week 4 you should have a clear picture of your estrogen response. If you are using an AI, do not crash E2. A reading below 20 pg/mL will kill your joints and your mood, and it will not help BP if the problem is volume. The goal is to keep E2 in the 30-50 range, not to zero it out.

Week 6 + Post-Cycle

End-of-cycle panel plus a week-10 follow-up. Dbol clears fast (half-life 3-6 hours) but the lipid and enzyme recovery takes 6-10 weeks. The week-10 draw tells you whether the cycle was a dents-or-damage event.

The week-10 follow-up is the number that tells you whether you can run Dbol again in the future. If ALT and GGT are back to baseline and HDL has recovered to within 10% of your pre-cycle value, you have a green light. If HDL is still 15 points lower, you have not fully recovered and you need more time. The same goes for BP: if it is still elevated at week 10, something else is going on, likely weight gain or a change in diet. Do not start another cycle until all markers are back to baseline. Dbol clears fast, but the metabolic stress it puts on your liver and cardiovascular system takes six to ten weeks to unwind. Respect that timeline and you can use it again. Ignore it and the next cycle will be worse.

Liver Support Is Real but Secondary

TUDCA (500 mg/day) genuinely lowers C17-AA enzyme elevation: the evidence is decent and the mechanism understood. But support stack does not change the lipid picture or the BP picture, and it cannot make a 10-week Dbol run safe. Support buys margin at the same dose; it does not buy dose escalation. The primary intervention is always shorter duration and lower dose.
